Reminder: FCC Filing Due November 2011

Please be reminded of the upcoming due date for the Report listed below:
Form 499-Q Quarterly Telecommunications Reporting Worksheet
Entities required to contribute to universal service support mechanisms must report their actual and projected end user and wholesale revenues for each calendar quarter by filing Form 499-Q on a quarterly basis.

Entities that believe they meet the de minimis exemption (annual contribution would be less than $10,000) from filing the Form 499-Q should complete the table contained in Figure 1 of the Form 499-Q instructions and retain Figure 1 and documentation of their contribution revenue base as such information must be provided to the FCC or Universal Service Administrative Company (“USAC”) upon request.

Providers of interconnected Voice over Internet Protocol service and commercial mobile radio service that rely upon traffic studies to report their revenues on the Form 499-Q must submit the traffic studies to the FCC.

The Form 499-Q filing for the third quarter 2011 is due to USAC by November 1, 2011.
