Please be reminded of the upcoming due date for the Report listed below:

Form 499-A Annual Telecommunications Reporting Worksheet
All intrastate, interstate and international providers of telecommunications within the United States, including interconnected VoIP providers, and payphone providers that are aggregators are required to file FCC Form 499-A with the Universal Service Administrative Company (“USAC”) each year with limited exceptions. Form 499-A is used to determine a company’s required contribution to the Universal Service Fund (“USF”), as well as to Local Number Portability (“LNP”) Administration, Numbering Administration (“NANPA”) and Telecommunications Relay Service (“TRS”), in addition to serving as a carrier’s FCC registration. The Form 499-A is due to USAC by April 1, 2011.

Note: The 2011 Form 499-A Instructions contain numerous revisions to last year’s Instructions. The Bureau has substantially reorganized the Instructions and consolidated some of the discussions. Any entity preparing to submit the Form 499-A by April 1, 2011 should carefully and thoroughly review the revised Instructions before completing and submitting the Form. Revisions to the Instructions will not be readily apparent this year, and likely will require a detailed comparison of the 2010 and 2011 Instructions to identify all of the changes.
